Our Duties
Luxora Medical, PLLC is required by law to maintain the privacy of protected health information ("PHI"), to provide you with this notice of our legal duties and privacy practices, and to abide by the terms of the notice currently in effect.
Permitted Uses and Disclosures
- Treatment. To coordinate care with physicians, pharmacies, and labs.
- Payment. To bill and collect payment for services rendered.
- Healthcare Operations. Quality improvement, credentialing, and internal audits.
- As Required by Law. Public health reporting, court orders, and similar obligations.
Uses Requiring Your Authorization
Most other uses and disclosures — including marketing, sale of PHI, and most disclosures of psychotherapy notes — require your written authorization, which you may revoke in writing at any time.
Your Rights
- Inspect and obtain a copy of your PHI.
- Request an amendment of your PHI.
- Receive an accounting of certain disclosures.
- Request confidential communications or restrictions on certain uses.
- Receive a paper copy of this notice.
- File a complaint without fear of retaliation.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with Luxora or with the U.S. Department of Health and Human Services, Office for Civil Rights.
