This Notice is effective as of the date shown above and applies to all patient records created or maintained by Luxora Medical, PLLC.

We understand that medical information about you is personal. We are committed to protecting your health information and are required by law to maintain the privacy of PHI, to provide you with this Notice of our legal duties and privacy practices, and to follow the terms of the Notice currently in effect.

• For Treatment — coordination with other providers, labs, and pharmacies.
• For Payment — billing and collection of fees.
• For Healthcare Operations — quality assurance, credentialing, training.
• Appointment Reminders — to remind you of scheduled visits.
• As Required by Law — including public health, law enforcement, and judicial orders.

• Right to inspect and copy your records.
• Right to request amendments.
• Right to an accounting of certain disclosures.
• Right to request restrictions on certain uses and disclosures.
• Right to request confidential communications.
• Right to a paper copy of this Notice on request.

We reserve the right to change this Notice and to make the new terms effective for all PHI we maintain.

If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer through the contact page or with the U.S. Department of Health and Human Services. We will not retaliate for filing a complaint.
